Enrollment

The first day your child attends the center, the following forms must be on file:

• Completed Application for Enrollment
• Up-to-date Children's Medical Report signed by a licensed physician
• Tuition agreement signed by parents
• Consent for Treatment
• Parent Handbook signature sheet signed by parents
